An unvented cylinder runs at mains pressure, which is what gives you a proper shower without needing a pump or a tank in the loft. It also means the work is notifiable and has to be done by someone holding the G3 qualification - that is a separate ticket from Gas Safe, and not every heating engineer has it. I do, so I can install, service and repair unvented systems and certify the work when it is finished.
If you already have one, it needs servicing every year. That is not just paperwork: the expansion vessel loses its charge over time, and the temperature and pressure relief valve and tundish have to be checked and working. Skip it and you can end up with a discharging valve, poor pressure, or a warranty the manufacturer will not honour. I check the lot, sort anything that needs it, and leave you with the certificate.
